Unchanged Procedures, Unexpected Occurrences: Unraveling the Mystery

  

In the realm of laboratory operations, we often encounter confusing situations where the tried-and-true procedures remain unchanged, yet unexpected occurrences arise. This begs the question: why does this happen?


Within the scientific community, a prevailing belief holds that diligently following the correct procedure, in line with standard operating procedure (SOP), should prevent such issues from arising. However, recent insights shed light on the notion that adherence to procedure alone may not always guarantee immunity from problems or occurrences.


Respected expert Clifford Chapman, MS, HTL (ASCP) QIHC, articulately addressed this paradox on June 8, 2021 in a thought-provoking blog titled “Troubleshooting Histology: Double Barrel Style- H&E and Special Stains". Chapman astutely posed the question: "What happened between yesterday and today?" He emphasized the significance of considering personnel changes or alterations in assigned tasks. Such factors highlight that expertise and the suitability of a procedure for a specific sample play crucial roles beyond the procedure itself.


Allow me to illustrate this challenge with a real-life example. On a day like any other in the laboratory, during my tenure as a supervisor, a pathologist requested a serial section from small biopsy. A diligent histology laboratory professional followed the correct procedure, and prepared the section from four consecutive ribbons. To our surprise, the stained sections exhibited varying thicknesses, defying our expectations. Despite adhering to the prescribed procedure, we found ourselves embarking on a meticulous investigation to uncover the root cause. Days of dedicated analysis ultimately led us to resolve the issue. It was the speed of the lever of the microtome. 


Undoubtedly, stress can arise when even meticulously followed procedures fail to yield the expected outcome, especially for supervisors and dedicated quality officers. However, it is crucial to establish a clear understanding of what constitutes the correct procedure before delving into problem-solving endeavors.


In the dynamic change of laboratory work, where unchanged procedures intersect with unforeseen occurrences, it is through careful examination and a holistic approach that we can unravel the mysteries that lie within, leading to enhanced scientific endeavors and improved outcomes.
